Your First Visit

Patient Forms

Upon entering our office, our receptionist will welcome you as a member of our family. We will request that you complete our patient forms located in the New Patient Center. We ask that you either print and complete the new patient forms prior to arriving at pour office. Or arrive 15 minutes prior to your scheduled appointment to complete the new patient forms in our office. This paperwork provides us with your health history and information on your condition.

Consultation

Next, you will have a consultation with Dr. Jerusha Franzluebbers, Dr. Veronica Roman or Dr. Luke Eikmeier to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for our team to learn more about you, your condition, and expectations to determine how we can help you reach your health and wellness goals.

After your consultation, Dr. Jerusha Franzluebbers, Dr. Veronica Roman or Dr. Luke Eikmeier will perform a complete chiropractic examination including a posture exam, palpation exam, spinal range of motion, SEMG, thermal scan, and  pulse wave profile.  Other standard neurological, orthopedic and physical tests may be performed as well. However, nothing will be done in our office without your consent.

Fremont Chiropractic Clinic uses state of the art Insight equipment during your initial  exam. You will receive a core score or a neurological report card. These reports are easy to understand and printed for you to take home and share with your friends and family if you so choose.

X-Ray Studies

Depending on your specific condition, we may refer you to have spinal x-rays taken. X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Most people are amazed once they see their X-rays up close. In some cases, they can even identify their misalignments and degeneration themselves.

Report of Findings

Once the information is collected and examinations are performed, Dr. Jerusha Franzluebbers, Dr. Veronica Roman or Dr. Luke Eikmeier will give you a detailed report of all the exam findings and answer any questions including:

  1. How can you help me?
  2. How often do I need to come in?
  3. What will my treatment cost?

After reviewing your health history, goals, and examining your spine and X-rays the doctors will discuss recommendations and notify you if your condition requires care with other providers. Dr. Jerusha Franzluebbers, Dr. Veronica Roman or Dr. Luke Eikmeier  will provide the best treatment and wellness program for your needs.

Treatment

At the conclusion of the report of findings you can choose to experience your first adjustment. It may also include NAET, Emotion Code, Quantum Neurology or Nutritional Support. Treatment is interactive, so you can express concerns about the different styles of treatment anytime.

Self Care Recommendations 

Prior to leaving, the doctor will suggest a wellness program to incorporate outside of treatment. If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches. If you desire, our wellness team will work with you also to create healthy habits and routines for your lifestyle. Every person is unique, therefore everyone requires a customized wellness plan.
